Does anyone know anything about these thermal waters. I know some of the spas in italy and thermal waters are therapeutic and need doctors recommendation for treatments etc.

Chanciano terme seems like a good option for stop prior to pienza but can't work out if we can spend a few hours enjoying the waters there, or not.

It has been awhile since I was there, but this is not the type of spa town you are wanting. This is more a medical spa.

Saturnia, in southern Tuscany, would be a better choice. They have a beautiful spa that is open to the public and the town is lovely.

Or, closer to Pienza, San Casciano dei Bagni. Friends of ours had a spa day in the latter (about 10 years ago) and liked it.
