I took a great guided tour Thursday night of Rome's Colosseum under the moonlight, which included an underground visit. It was wonderfully atmospheric and incredibly calm and still inside the massive structure (the resident black cat Nero was also prowling around!)
The tours run throughout the summer until October several times each evening on Thursday and Saturday, starting at about 8:30. The price was a bit of an ouch - 20 euro - but I think it was worth it.
For anyone with accessibility issues, the glass elevators were running, one to the underground level and a separate lift to the first level (I had shredded a knee and those huge stone steps inside the Colosseum are daunting!)
I went with a friend who is a tour guide, and we found there were no lines, no crowds, so I don't think an advance ticket is necessary.
